This brought to mind training with live ammunition in Northern Namibia in the 70's - at end of day roll call one of the regiment had gone missing - a search found his rifle with a blown magazine lying in the veld and some blood, but no soldier. A day later we found a dazed and fortunately lightly injured troopie walking home to Cape Town - he had had enough of that game

Turns out we had a faulty batch of ammo - when being extracted, the back end of the casing could rip off and leave the casing in the breach which then caused improper closure of the breach which damaged the next round causing the powder to ignite and blow down into the magazine setting off some rounds. Needless to say we didn't feel that confident in our equipment after that.
